You should only fill out the form if you intend to take part in i-PARACHUTE 2010. 

Islamic Relief Scotland's  i-PARACHUTE 2010, a sky dive takes place on 17th July 2010 in Fife. To take part you will need to pay a non refundable deposit of £150 and commit to raising a minimum sponsorship amount of £500.

Please note that every person taking on the challenge needs to fill in his or her own form. Once we’ve received your application form we will call you to confirm registration and arrange for payment of deposit.
